Making a Difference by Giving Back

Community involvement at Endo is a living, breathing measure of how we make a difference. It's about combining employee involvement with strong organizational support to make things happen. We know that by banding together as a community within Endo, we can create a positive impact on the greater communities where we live and work. As a way to get our employees actively involved, Endo organizes regular volunteer activities, offers an employee community involvement grants program and matches employees' donations up to $1,000 per year.

The Corporate Giving program primarily supports initiatives and organizations that are aligned with our goals of improving patient care and outcomes. Endo is committed to improving the communities where we live and work by supporting organizations that have a direct impact on helping people in need access basic necessities, healthcare and education. 2011 CORPORATE GIVING PROGRAM

Overview and Giving Focus Areas

The program primarily supports initiatives and organizations that are aligned with our goals of improving patient care and outcomes for the conditions we treat.

Endo is committed to improving the communities where we live and work by supporting organizations that have a direct impact on helping people in need access basic necessities, healthcare and education.

Our corporate giving program focuses on supporting programs and organizations in the following areas:

  • Improving patient care.
    • Provide services and resources to help patients and their families cope with their condition/disease (excludes patient education programs)

  • Literacy & Science Education
    • Literacy for children and adults
    • Provide access to science education to underprivileged children and young adults (Kindergarten through College)

  • Improving Our Communities
    • Provide basic necessities to people in need (e.g.,food, shelter, etc.)
    • Access to healthcare for the uninsured and underinsured

    Preference is given to requests focused on our therapeutic areas and/or support for communities near one of our locations. Our therapeutic areas are pain management, urology, oncology and endocrinology. Locations currently being considered for support are communities in Chadds Ford, PA, Westbury, NY, Cranbury, NJ, Austin, TX and Huntsville, AL.


Guidelines

Charitable donations are only provided to publicly funded organizations in the U.S. that are classified as an IRS 501(c)(3) tax-exempt nonprofit.

Types of requests that will not be considered are:

  • Membership dues (corporate or individual) to professional associations
  • Direct contributions to individuals (e.g., scholarship or hardship assistance)
  • Contributions to organizations existing primarily for political purposes
  • Funding for sectarian and religious organizations
  • Private foundations
  • International organizations and causes
  • Medical or clinical education grants
  • Memorial funds and labor organizations
  • Sports teams
  • Conferences
  • Patient education programs
  • Organizations or programs that discriminate on the basis of age, race, national origin, ethnicity, gender or disability
  • Requests that fall outside our giving focus

Important information for all applicants:

  • Any contribution should be considered a one-time commitment without guarantee of renewal (unless a multiple year commitment letter from Endo is received).
  • We prefer to support programs that will have a direct impact rather than sponsoring events and fundraising galas.
  • Requests are only considered for the current calendar year.
  • Limit one corporate contribution application, per organization, per calendar year.
  • Organizations that receive a grant of $50,000 or more will be required to submit a final report (11 months after grant or at the project end date, if earlier) before a new request is considered.

All funding decisions are at the sole discretion of Endo Pharmaceuticals and may not be appealed. Endo reserves the right to end the program without notice to applicants.

How to Apply for a Corporate Contribution

Educational grants are not funded through the Corporate Giving Program. To submit an educational grant request, please visit the grant request website: www.EndoRequests.com.

All requests must be submitted by the organization through the online request form.

Faxed, mailed, e-mailed or phoned requests will not be considered.

Application submission does not guarantee funding.

Please note the following documents are required to be submitted with the online application.

  • Completed and signed W-9 form (organization name and address must match online application)
  • Most recent IRS 990 tax form and audited financial statements are required for requests of $10,000 or more.
  • Event information, including all sponsorship levels (if applying for event sponsorship)
